You have the opportunity to get guidance on community activities through a wellbeing referral. A wellbeing referral is a referral made by a social or health professional to a service that links third sector and leisure services to support your wellbeing. Through a wellbeing referral, the client is assigned a link person with whom they can plan and implement wellbeing and inclusion activities together.
